Current Medications
If you are currently taking any medications (including oral contraceptives), supplements or powders (i.e protein powder), please SMS Kerry on 0418294295 with the names. This will save valuable time during your consult.

Health Appraisal Questionnaire
As a further means of saving consult time in the Clinic, I will email you a Health Appraisal via the Metagenics Website.

Follow Up Appointments
For follow up appointments, please bring all of your Practical Health prescribed Supplements and/or Homeopathics to test for their effectiveness.
